We are currently welcoming new participants statewide who are 18 years or older and eligible to receive Developmental Disability Services. 

Our dedicated Targeted Service Coordinators (TSCs) will work with you and your support team to develop a personalized support plan. This plan ensures that your services are not only tailored to your goals but also monitored and maintained throughout each plan year.
